People Care of New Jersey to Assist Seniors at Potomac Homes, Heritage Pointe of Teaneck

TEANECK, N.J., July 15, 2009 – People Care of New Jersey, a comprehensive geriatric evaluation and home health agency based in Hackensack, New Jersey, has joined together with Potomac Group Homes and Heritage Pointe of Teaneck to offer a continuum of care for seniors in northern New Jersey and surrounding areas.

Potomac Group Homes provides a residential alternative to institutional care for individuals with Alzheimer’s disease and related dementia at various locations throughout New Jersey, including Chester, Cresskill, Green Brook, two in Hillsdale, Mahwah, Montville, Paramus, Park Ridge, Princeton, Ramsey and Woodcliff Lake.

Heritage Pointe of Teaneck is a full-service independent senior living community, situated in the heart of Bergen County. Bordered by 49 acres of natural woodlands, yet convenient to all of the culture and recreation of Manhattan, the community offers luxurious apartments, distinctive amenities, spa membership and fine dining for active seniors.

As part of the new alliance, professionals and nurses at People Care will provide resident assessment, family outreach and coordinate home health services for residents at Potomac and Heritage Pointe of Teaneck. The partnership also will provide seniors with consistent and specific care to “age in place” throughout their retirement years. Residents will see the same faces that are very familiar with their individual needs and nuances making it seamless both for them and their loved ones according to Benjamin Pearce, chief executive officer of Potomac Homes. People Care also will provide residents with wellness information, resources and education to better achieve a healthy lifestyle.

In addition to home health care, People Care offers a complete nursing assessment at no cost to families. “We routinely send out our nurses to meet with families in their own homes so that a comprehensive assessment can be conducted in the individual’s living environment,” stated Donna Sorge, RN, BSN, executive director of People Care of New Jersey. “In this way, we can all be informed about an individual’s functional status and safety in their home environment.
